Position Description:

Harbor Health operates two Programs for All-Inclusive Care for the Elderly in Mattapan and Brockton (PACE Programs), providing comprehensive healthcare, transportation and social services for more than 580 frail elders who continue to live with dignity and independence in the community. We are currently seeking an extremely talented Nutrition Manager to join our team at the Elder Service Plan in Mattapan and Brockton.

Role:
The Nutrition Manager for Elder Service Plan (ESP) reports to the Director of Clinical Services and is responsible for developing and implementing the overall nutrition program for the Elder Service Plan in accordance with PACE (Program for All-Inclusive Care for the Elderly), DPH and other compliance protocol, assisting with Nutrition Dept. budgets, strategic planning, overseeing all Nutrition staff and participating in compliance audits. Position includes oversight of Nutritional services at both of our ESP locations in Mattapan and Brockton and may include home care visits as needed.

Responsibilities:
• Develops, coordinates and implements nutrition programs for ESP; ensuring that the nutrition programs are in compliance with regulatory and compliance agencies.
• Responsible for all aspects of Nutrition Department personnel including hires, develops, evaluates, trains, and supports the Nutrition staff, supervises the Dietary Aides and other Dieticians and ensures staff ratios are on-par with participant/program demands.
• Oversees Dietician performance – ensures assessments are completed timely and comprehensive; develops protocols and tools to support population management; ensures Care Plans are updated accurately.
• Ensures that all meals are in compliance with PACE and DPH requirements.
• Provides guidance and advice to participants and their families regarding nutrition and provides nutritional educational programs and materials to participants, families and ESP staff.
• Oversees the food services programs provided by the food vendors.
• Actively participates in the interdisciplinary teams; covers 20% panel and/or escalation cases.
• Continuously improves Nutrition operations and quality (e.g., supplement management, vendor management for food satisfaction).
• Performs initial, semiannual, and annual nutritional assessments.
• Ensures the Dietary Aides are performing all necessary compliance functions.
• Provides or supports Kitchen coverage when needed.
• Places and/or manages orders as needed: reviews and approves all nutrition-related invoices.
• Develops and implements Annual Safety Training for ESP Staff.
• Assists in the development, implementation and management of the Nutrition budget.
• Prepares and actively participates in compliance audits.
• Participates in ESP strategic planning
• Must be able to travel between Harbor ESP sites and participants residence and other outside sites as needed in a timely manner.

Minimum Qualifications:

Requirements:
• Bachelor’s Degree in Nutrition/Dietetics, Master’s degree in Public Health Nutrition, Foods and Nutrition or Master’s degree in Nutrition and Dietetics preferred.
• Currently Licensed in the state of Massachusetts as a Registered Dietician
• Current Serve Safe Manager Certification, MA Food Allergen Certification required (or obtained within 3 months of hire) and current BLS (Basic Life Support for Healthcare Providers) Certification required
• Two years of professional experience providing nutritional counseling and assessments of which one year has been as a direct or functional supervisor of nutrition staff.
• One year of experience working in geriatrics required.
• Excellent verbal and written communication skills.
• Intermediate skills with Office 365 including Word, Excel and Outlook.
